Accident investigation? Lol....!! it's so easy :)

An accident happened. Leakage, spillage, everything happened as usual. People were busy.
The concerned engineer was playing candy crush on his mobile. He couldn’t care for the message received on his wireless that had been set to low volume. He was busy.
The ambulance driver went outside to buy gutka. His boss needed gutka. He was busy too.
People gathered around. They started discussing. They turned busy too.
Where are the certified first aiders? They were shocked too. They had been shown demo on stage three years ago and issued certificates. They couldn’t help too.
The person is lying down, but now arise the question – Where is his buddy? Sorry, the buddy is in canteen sipping his cup of tea. He didn’t know too.
The buddy should be happy. He didn’t have to see the person injured.
Oh God…!!! Who is the buddy and who has been injured? The technician is at the canteen and it was his helper injured at work.
The man was taken to the Health centre in a bolero campher.
Fighting all odds he survived.
The HSE team knew about the incident in two days.
Now there will be an investigation - Accident Investigation.
An investigation team was formed. The team designated to investigate consisted of honchos from five different departments (Namely finance, HR, admin, commercial and Maintenance)
 Investigation started the very next week. The committee decided to visit the accident site. Location? Where did the accident happen? Was the accident real or a fabricated one? One pointed to the right, another to the left. Finally the committee moved straight and reached the location with the help of a guide. To their pleasure everything was back to normal. (definitely it calls for a party – everything is back to normal and this shows that the accident didn’t have any effect on the business….!!! Happiness overflowed). The team took twenty five different photographs from fifteen different angles. An eye witness appeared from nowhere. Gave a statement before the investigation committee – ‘ The person didn’t wear PPEs’.
The committee made a thirty page report which says

Root cause- The person was reluctant to wear PPEs

Corrective action – Terminated him 

Tail end: The team was congratulated by the group CEO for their findings

    1. Yes definitely, safety is taken seriously :) I know I have exaggerated a bit.
      I would love to believe that 'Safety is a continuous journey; it's not a destination'. I want more. Dil more maangta heh yaar:P

  3. U have said universal truth no doubt . And main thing is in safety we should co-operative with each other many places which I have seen their team members use to be have like safety representative came from out of space ... Heehehe
